Elton John welcomes expansion of opt-out testing for HIV to 46 Accident & Emergency sites across England and calls on all political leaders to do more to end AIDS in a speech at Speaker’s House
Elton John: "HIV is not yesterday's story. It should be a priority today."
LONDON, Nov. 29, 2023 /PRNewswire/ -- Elton John was honoured at a reception hosted by the All Party Parliamentary Group on HIV/AIDS at the Speaker's House today in recognition of his enduring commitment to ending the AIDS epidemic, both personally and through the work of the Elton John AIDS Foundation.
Earlier today, Victoria Atkins, Secretary of State for Health and Social Care outlined how the UK Government will be expanding the hugely successful NHS opt-out blood-borne virus testing programme for HIV and Hepatitis to 46 new emergency departments across England. Expansion of the programme, from the current 33 sites to every high prevalence area, could identify a significant proportion of the estimated 4,500 people living with undiagnosed HIV, prevent new transmissions and save more lives.
Addressing a packed Speaker's House, Elton John welcomed the UK Government's further roll-out of opt-out testing: "My Foundation and partners did a three-year pilot (2018-2021) in the South London boroughs of Lambeth, Southwark and Lewisham. This pioneering approach showed beyond a doubt that opt-out testing works in A&E. Automatic testing gets to people earlier, which means less HIV transmission, less illness, less death and by the estimate of health economists, £50 million saved for the NHS. So, to hear today from the Secretary of State that this work will be expanded to every high HIV prevalence area…46 new Accident & Emergency departments in local hospitals across England…more than doubling the number of HIV tests, is truly wonderful news."
He also shared his personal reflections on the long fight to end the AIDS epidemic and called on all political leaders to do more: "Whoever wins the next general election can help to end AIDS worldwide by 2030. With 39 million people around the world living with HIV and 1.3 million people newly infected last year, Elton John also spoke about the need for the UK to do more internationally: restoring the UK's full commitment to The Global Fund to fight AIDS, TB and Malaria, establishing new models of partnership and engagement, and protecting civil liberties.
"Abandoning ending AIDS wouldn't just be a failure for the 39 million people living with the disease. We'd be failing to prepare for whatever global virus comes next, because we know that fighting AIDS is pandemic preparedness in action. So, we should be increasing our commitment to the Global Fund, not cutting it. And if the Government is serious about ending AIDS, I call on it to fully restore the UK's commitment, to pledge £1.46 billion pounds this current three-year cycle and ensure the Global Fund is fully financed."
World AIDS Day will be commemorated on 1st December and despite advances in treatment, 630,000 people died of AIDS related diseases in 2022. Worldwide, HIV rates are rising faster among young people than in any other group and more than half of people living with HIV are women and girls. Over nine million people are not currently accessing treatment that could save their lives, all too often because shame and stigma is holding them back.

FIRENZE, Italia, e NEW YORK, 9 dicembre 2023 /PRNewswire/ -- Il Gruppo Menarini ("Menarini"), azienda farmaceutica e diagnostica leader a livello internazionale, e Stemline Therapeutics, Inc. ("Stemline"), una società interamente controllata del Gruppo Menarini focalizzata nell'offerta di terapie oncologiche trasformative ai pazienti affetti da tumori, oggi hanno presentato i risultati di una nuova analisi post-hoc dello studio clinico pivot EMERALD, che ha dimostrato un miglioramento clinicamente significativo nella sopravvivenza libera da progressione (PFS) in tutti i sottogruppi rilevanti. I dati mostrano una PFS favorevole per ORSERDU® (elacestrant) in monoterapia rispetto allo standard di cura (SOC) nei pazienti con carcinoma mammario metastatico (mBC) o avanzato ER+, HER2-, affetti da tumori endocrino-sensibili e con mutazioni di ESR1, a seguito di un precedente trattamento con inibitori delle CDK4/6 di almeno 12 mesi. EMERALD è uno studio registrativo di fase 3 che ha dimostrato una PFS statisticamente significativa ottenuta con ORSERDU rispetto alla monoterapia endocrina SOC (fulvestrant, letrozolo, anastrozolo ed exemestane). In base a questi risultati, il 27 gennaio 2023 la FDA ha approvato ORSERDU per il trattamento di donne in postmenopausa o di uomini adulti con carcinoma mammario avanzato o metastatico con ER+, HER2-, mutazioni di ESR1 e progressione della malattia dopo almeno una linea terapica endocrina.Fino al 40% dei pazienti ER+, HER2- con malattia avanzata o mBC presenta mutazioni di ESR1. Si tratta di un fattore noto di resistenza alla terapia endocrina standard e, finora, il trattamento dei tumori che presentano queste mutazioni è stato più difficile.
Va sottolineato che una precedente analisi post-hoc su sottogruppi dei risultati PFS di EMERALD, presentata a SABCS 2022, ha dimostrato che la durata di un precedente trattamento con inibitori delle CDK4/6 era positivamente associata a una PFS più estesa con ORSERDU, ma non con SOC. Nei pazienti con mutazioni di ESR1 trattati con inibitori delle CDK4/6 per almeno 12 mesi prima della randomizzazione in EMERALD, ORSERDU ha raggiunto una PFS mediana di 8,6 mesi rispetto a 1,9 mesi del SOC, con una riduzione del 59% del rischio di progressione o morte (HR=0,41; IC 95%: 0,26-0,63). In questa analisi aggiornata, Menarini Stemline ha valutato il beneficio di ORSERDU in monoterapia in sottogruppi clinici e di biomarcatori chiave ad alta prevalenza, inclusi i pazienti con metastasi ossee, epatiche e/o polmonari; quelli con comuni mutazioni concomitanti comuni di PIK3CA o TP53; oppure quelli con bassa espressione di HER2.
"Questi risultati aggiornati confermano ulteriormente che ORSERDU in monoterapia rappresenta un'opzione di trattamento di seconda linea promettente per i pazienti ER+, HER2- con carcinoma mammario metastatico e mutazioni ESR1", è il commento della Dott.ssa Virginia Kaklamani, DSc, oncologa specializzata in tumori mammari e docente di medicina presso la UT Health San Antonio, MD Anderson Cancer Center. "Abbiamo registrato importanti miglioramenti nella sopravvivenza libera da progressione in molti sottogruppi importanti con il trattamento a base di elacestrant in monoterapia rispetto allo standard di cura nei pazienti trattati per almeno 12 mesi con una terapia a base di CDK 4/6. Abbiamo osservato questi risultati non solo per quanto riguarda le metastasi ossee, ma anche per le metastasi epatiche e/o polmonari, e nei pazienti con co-mutazioni comuni, come PIK3CA e TP53, oltre che in pazienti con bassa espressione di HER2".
ORSERDU ha dimostrato un miglioramento clinicamente significativo nella PFS rispetto alla monoterapia endocrina SOC (fulvestrant, letrozolo, anastrozolo ed exemestane) in tutti questi sottogruppi. ORSERDU ha dimostrato una PFS significativamente superiore a seguito di un trattamento precedente con inibitori delle CDK4/6 della durata di almeno 12 mesi, il che indica che quando i tumori con mutazione di ESR1 restano sensibili al sistema endocrino, il pathway ER potrebbe rappresentare un fattore chiave della malattia, indipendentemente dal sito metastatico, dalle mutazioni concomitanti di PIK3CA o TP53, o ancora dalla bassa espressione di HER2. Informazioni sullo studio di fase 3 EMERALD (NCT03778931)Lo studio EMERALD è uno studio di fase 3, randomizzato, in aperto e con controllo attivo che ha valutato elacestrant somministrato come monoterapia di seconda o terza linea nei pazienti ER+, HER2 con malattia in stadio avanzato/mBC. Per lo studio sono stati arruolati 478 pazienti che avevano precedentemente ricevuto una o due linee di terapia endocrina, incluso un inibitore delle CDK4/6. I pazienti arruolati nello studio sono stati randomizzati per ricevere elacestrant o un agente ormonale approvato a scelta dello sperimentatore. Gli endpoint primari dello studio erano la sopravvivenza libera da progressione (PFS) sia nella popolazione complessiva di pazienti, sia nei pazienti con mutazioni del gene del recettore estrogenico 1 (ESR1). Informazioni importanti di sicurezza Avvertenze e precauzioni
Dislipidemia: ipercolesterolemia e ipertrigliceridemia si sono verificate in pazienti trattati con ORSERDU con un'incidenza rispettivamente del 30% e del 27%. L'incidenza di ipercolesterolemia e di ipertrigliceridemia di grado 3 e 4 è stata rispettivamente dello 0,9% e del 2,2%. Monitorare il profilo lipidico prima di iniziare l'assunzione di ORSERDU e periodicamente durante il trattamento. Tossicità embrio-fetale: In base ai risultati sugli animali e al suo meccanismo d'azione, ORSERDU può causare danni al feto se somministrato durante la gravidanza. Informare le pazienti in gravidanza e in età riproduttiva del possibile rischio per il feto. Consigliare alle donne in età riproduttiva di utilizzare metodi contraccettivi efficaci nel corso del trattamento con ORSERDU e per 1 settimana dopo l'ultima dose. Reazioni avverse
Reazioni avverse gravi si sono verificate nel 12% dei pazienti trattati con ORSERDU. Le reazioni avverse gravi in >1% dei pazienti trattati con ORSERDU sono state dolore muscolo-scheletrico (1,7%) e nausea (1,3%). Reazioni avverse fatali si sono verificate nell'1,7% dei pazienti trattati con ORSERDU, tra cui arresto cardiaco, shock settico, diverticolite e cause sconosciute (un paziente per ciascuna reazione).Le reazioni avverse più comuni (>10%) con ORSERDU, incluse anomalie di laboratorio, sono state: dolore muscolo-scheletrico (41%), nausea (35%), aumento del colesterolo (30%), aumento dell'AST (29%), aumento dei trigliceridi (27%), affaticamento (26%), diminuzione dell'emoglobina (26%), vomito (19%), aumento dell'ALT (17%), diminuzione del sodio (16%), aumento della creatinina (16%), diminuzione dell'appetito (15%), diarrea (13%), cefalea (12%), costipazione (12%), dolore addominale (11%), vampate di calore (11%) e dispepsia (10%).
Interazioni con farmaci
Uso concomitante con induttori e/o inibitori della CYP3A4: evitare l'uso concomitante di inibitori forti o moderati di CYP3A4 con ORSERDU. Evitare l'uso simultaneo di induttori forti o moderati della CYP3A4 con ORSERDU.
Uso in popolazioni specifiche
Allattamento: avvisare le pazienti di non allattare al seno nel corso del trattamento con ORSERDU e per 1 settimana dopo l'ultima dose.Insufficienza epatica: Evitare l'uso di ORSERDU nei pazienti con insufficienza epatica grave (Child-Pugh C). Ridurre il dosaggio di ORSERDU nei pazienti con insufficienza epatica moderata (Child-Pugh B).Non sono state ancora stabilite la sicurezza e l'efficacia di ORSERDU nei pazienti pediatrici.

ELCIN (NCT05596409) è uno studio di fase 2 sulla valutazione dell'efficacia di elacestrant nei pazienti con carcinoma mammario avanzato/metastatico positivo per il recettore degli estrogeni (ER+)/negativo per il recettore 2 del fattore di crescita epiteliale umano 2 (HER2-) sottoposti a una o due precedenti terapie ormonali, ma non a un precedente inibitore della chinasi ciclina-dipendente mirato agli enzimi CDK4 e CDK6 (CDK4/6i) in ambito metastatico. Chinese Ambassador to Vietnam Xiong Bo told China Media Group that there are many favorable conditions for people-to-people exchanges between China and Vietnam.
With similar historical and cultural heritages, Vietnamese people are very familiar with Chinese history and Confucianism, and contemporary Chinese films as well as TV series are very popular among Vietnamese, he said.
To boost mutual understanding and friendship between the two peoples, China and Vietnam have emphasized the importance of work on educational cooperation.
Zheng Dawei, education counselor of the Chinese Embassy in Vietnam, said over 50 higher education institutions in Vietnam offer Chinese language departments or majors, with 15,000 students studying Chinese at the higher education level.
According to a joint statement on deepening China-Vietnam comprehensive strategic cooperative partnership issued last year, China announced that in the next five years, it will offer Vietnam over 1,000 Chinese Government Scholarships and more than 1,000 International Chinese Language Teachers Scholarships, helping Vietnam train high-level talents and Chinese language teachers.
From 2021 to 2022, over 27,000 Vietnamese students studied in China, with more than 1,700 of them receiving Chinese Government Scholarships, said Ambassador Xiong.
Xiong said the foundation of public opinion between both sides needs to be further strengthened. The two sides should place greater importance on strengthening the China-Vietnam friendship, he noted.
More people-to-people exchanges expected
After the 20th National Congress of the Communist Party of China, General Secretary of the Communist Party of Vietnam Central Committee Nguyen Phu Trong was the first foreign leader invited for a visit to China.
At the invitation of Trong and Vietnamese President Vo Van Thuong, Chinese President Xi Jinping will pay a state visit to Vietnam from December 12 to 13.
The leaders will discuss upgrading China-Vietnam relations and focus on six major areas – politics, security, practical cooperation, public support, multilateral affairs and maritime issues – Chinese Foreign Ministry spokesperson Wang Wenbin said at a regular press briefing.
